Key Responsibilities:

Virtual Assessment: Conduct remote, client-centered functional assessments of the home environment using our app platform and client input.

Modification Planning: Develop detailed, practical, and client-specific recommendations for home modifications, durable medical equipment (DME), and assistive technology (AT) to enhance safety, independence, and participation.

Documentation & Reporting: Maintain timely and accurate documentation of evaluations, treatment plans, and progress notes in compliance with all relevant state, federal, and professional standards.

Collaboration: Consult and coordinate with clients, caregivers, family members, installers, local resource providers, and the Accessibility NextGen team to ensure successful implementation of recommended modifications.

Education: Provide education and training to clients and caregivers on the safe and proper use of recommended equipment and modified home features.
